Cardiovascular

Hexedrone's sympathomimetic action places acute stress on the cardiovascular system. A fatal NEH case (Kovács et al. 2019) explicitly identified pre-existing cardiac pathology — childhood arrhythmia, hypertrophic/dilated cardiomyopathy, severe atherosclerosis — as a significant co-factor in the fatal outcome at 285 ng/mL postmortem blood concentration.
Neurological

Stimulant cathinones may lower seizure threshold. The patient in the Kovács (2019) NEH fatality had a history of seizures, though the direct contribution of NEH to seizure risk was not isolated. Class-level caution applies.
Psychiatric

Chronic cathinone use is associated with acute psychosis, hypomania, paranoid ideation, and delusions (Weinstein et al. 2017). Hexedrone's high DAT selectivity, comparable to cocaine, confers significant psychosis risk with heavy or binge-pattern use. Post-intoxication psychiatric symptoms including anxiety, paranoia, and anhedonia were documented in the NEH case series (Lefeuvre 2024).
Renal

Acute kidney failure was documented in the Domagalska (2021) fatal NEH case. Pre-existing renal impairment may lower the threshold for renal complications during sympathomimetic toxicity. Evidence is limited to a single case report from the NEH analog.
Pregnancy & Breastfeeding

No reproductive toxicity data exists for hexedrone or NEH. Cardiovascular and thermal stress from sympathomimetic stimulation poses theoretical risk to fetal development. Standard precautionary contraindication for all stimulant cathinones.

Concurrent use of monoamine oxidase inhibitors with hexedrone risks hypertensive crisis through prevention of catecholamine breakdown, leading to dangerous accumulation of norepinephrine and dopamine. This interaction is potentially fatal and is contraindicated for all cathinone-class stimulants.
