Contraindications

Cardiovascular

  • pre-existing cardiovascular diseaserelative

    Tachycardia documented at 110 mg (Shulgin). Sympathomimetic cardiovascular stress is compounded by PMMA's hyperthermic potential. Pre-existing cardiac conditions increase risk of arrhythmia and cardiovascular collapse.

  • hypertensionrelative

    PMMA's NET-mediated norepinephrine release produces blood pressure elevation. Pre-existing hypertension increases risk of hypertensive crisis or cardiovascular events.

Psychiatric

  • MAOI therapyabsolute

    Combination of PMMA's endogenous MAO-A inhibition with pharmaceutical MAOI therapy creates additive MAO blockade on top of PMMA's serotonin-releasing activity. Serotonin syndrome is near-certain.

  • SSRI/SNRI therapyabsolute

    PMMA's serotonin-releasing and MAO-A-inhibiting mechanisms combined with reuptake inhibition produces extreme serotonergic overload. No PMMA-specific case reports located but class-inferred risk is severe.

Hepatic

  • hepatic impairmentrelative

    Reduced hepatic function may impair CYP2D6-mediated PMMA metabolism, leading to elevated blood concentrations and increased toxicity risk.

Renal

  • renal impairmentrelative

    PMMA is a substrate of OCT2 for renal excretion (Jensen 2020). Impaired renal function may reduce drug elimination, contributing to accumulation.

Metabolic

  • CYP2D6 poor metabolizer statusabsolute

    CYP2D6 poor metabolizers (~5-10% of European populations) cannot adequately clear PMMA, resulting in drug accumulation. Given PMMA's narrow margin between active and lethal concentrations, impaired clearance may be fatal.

  • hyperthyroidismrelative

    PMMA's sympathomimetic profile may interact dangerously with hyperthyroid states, both conditions independently producing tachycardia and elevated metabolic rate.

Pregnancy & Breastfeeding

    No specific PMMA teratogenicity data exist. Contraindicated on general pharmacological grounds as a sympathomimetic monoamine releaser with severe hyperthermic potential and documented lethality.
