Cardiovascular

Cardiovascular disease or uncontrolled hypertension. Lysergamides produce transient sympathomimetic effects including vasoconstriction, tachycardia, and mild hypertension. These effects may pose risks to individuals with pre-existing cardiovascular conditions.
Neurological

Concurrent lithium use. Combination of lithium with classical psychedelics has been associated with seizure risk in case reports. The mechanism is not fully characterized but is consistently reported as a class-level interaction for serotonergic psychedelics.
Psychiatric

Personal or family history of psychotic spectrum disorders. 5-HT2A receptor agonism by lysergamides can trigger acute psychotic episodes in individuals with genetic or clinical predisposition to psychosis. This is a well-established class-level contraindication for all serotonergic psychedelics.
- Prior HPPD episodes or predisposing factorsrelative

Prior HPPD episodes or predisposing factors. Hallucinogen persisting perception disorder is a rare but documented condition following hallucinogen use. Predisposing factors include pre-existing anxiety, prior HPPD episodes, tinnitus, visual floaters, and concentration difficulties (Halpern et al. 2018).
Hepatic

Severe hepatic impairment. If PARGY-LAD metabolism proceeds via CYP1A2 and CYP3A4 as established for structural analogs AL-LAD and ETH-LAD (Wagmann et al. 2019), hepatic impairment may significantly alter clearance and increase exposure duration or intensity.

Concurrent use of monoamine oxidase inhibitors (MAOIs). MAOIs prevent serotonin breakdown, and combining them with a serotonergic psychedelic creates a pharmacologically plausible pathway to life-threatening serotonin toxicity syndrome. This is a class-wide critical hazard for all lysergamides.

Concurrent tramadol use. Tramadol acts as a weak serotonin reuptake inhibitor in addition to its opioid agonist activity. Combining tramadol's serotonin reuptake inhibition with PARGY-LAD's 5-HT2A agonism and presumed 5-HT1A activity creates a pharmacologically plausible risk for serotonin toxicity syndrome.
