Pre-existing hypertension, arrhythmias, heart failure, or other significant cardiovascular disease. Cardiovascular stimulation at higher doses may exacerbate these conditions.

Personal history of schizophrenia spectrum disorders or other primary psychotic disorders. 5-HT2A agonism disrupts thalamocortical gating and may trigger psychotic decompensation.

Personal history of bipolar I disorder. Risk of manic episode triggering. Bipolar II may carry relative rather than absolute risk, but caution is warranted.

Significant hepatic impairment may reduce MiPT clearance and increase exposure. No substance-specific data; inferred from CYP-dependent metabolism of analog compounds.

Pregnancy and breastfeeding. No reproductive safety data for MiPT or closely related tryptamines.

Concurrent use of monoamine oxidase inhibitors including pharmaceutical MAOIs (phenelzine, tranylcypromine, moclobemide) and beta-carboline MAOIs in ayahuasca. Serotonin syndrome risk is severe.

Concurrent lithium therapy. Multiple case reports associate lithium + psychedelic combinations with seizures across the serotonergic psychedelic class.

Concurrent SSRI or SNRI use. Risk is moderate rather than severe due to MiPT's lack of SERT inhibition, but serotonin syndrome remains possible.
