Contraindications

ConditionSeverityNote
Respiratory
Respiratory insufficiencyabsolute

Methaqualone produces dose-dependent respiratory depression as its primary life-threatening effect. Pre-existing respiratory insufficiency or disease lowers the margin of safety substantially.

Neurological
Seizure disordersabsolute

Methaqualone overdose can paradoxically produce convulsions despite being a GABAA potentiator. Additionally, withdrawal from chronic use lowers seizure threshold. Individuals with pre-existing seizure disorders face elevated risk from both acute toxicity and withdrawal.

Psychiatric
History of substance use disordersrelative

Methaqualone has documented high abuse liability — controlled comparison found significantly elevated euphoria and abuse-liability scores versus placebo and diazepam. Compulsive redosing urge is a characteristic feature. Individuals with a history of substance use disorders face elevated risk of dependence.

Hepatic
Hepatic impairmentrelative

Methaqualone is >95% hepatically metabolized via CYP3A4. Impaired liver function prolongs drug exposure, increasing both the intensity and duration of effects and the risk of toxicity.

Pregnancy & Breastfeeding
Pregnancyrelative

Methaqualone crosses the placenta. Pharmacokinetics are altered during pregnancy and labour, with half-life prolonged during labour and shortened during active pregnancy compared to non-pregnant controls.

Other
CNS depressant co-useabsolute

Concurrent use of any CNS depressant — including benzodiazepines, barbiturates, GHB, neurosteroids, and anesthetic agents — with methaqualone produces additive or synergistic respiratory depression risk. The mechanism is pharmacodynamic: convergent GABAA potentiation from multiple modulatory sites. This combination is potentially lethal.

Alcohol use (within 3 days)absolute

Alcohol synergizes with methaqualone through pharmacodynamic sensitization that persists for at least 3 days after a single therapeutic dose. Ethanol reduces the minimum effective dose of methaqualone and separately decreases its elimination rate. In mice, methaqualone prolonged ethanol-induced narcosis by 269%. Individuals who have taken methaqualone may be unaware that their sensitivity to alcohol remains elevated for days afterward.

Baclofenabsolute

Baclofen is a GABAB agonist that independently suppresses respiratory drive. Combined with methaqualone's GABAA enhancement, the result is additive CNS and respiratory depression. The seed data classifies this interaction as lethal. No specific published case report was identified, but the pharmacodynamic mechanism is well-supported.

Opioid co-useabsolute

Opioids suppress respiratory drive via mu-opioid receptors in the brainstem respiratory center — a pharmacodynamically distinct pathway from GABAA modulation. Combined with methaqualone, the result is multiplicative respiratory depression risk.

CYP3A4 inhibitorsrelative

Drugs or substances that inhibit CYP3A4 — including ketoconazole, erythromycin, and grapefruit juice — would be expected to increase methaqualone plasma concentrations and prolong exposure, increasing toxicity risk. This contraindication is inferred from metabolic pathway data, not from direct interaction studies.
