Cardiovascular
- uncontrolled hypertensionrelative
Uncontrolled or poorly controlled hypertension. Klaiber et al. 2024 documented significant BP elevation at all doses above 100 mg.
- cardiovascular diseaserelative
Coronary artery disease, heart failure, arrhythmias, or other structural cardiac disease.
Psychiatric
- psychotic disordersabsolute
Personal history of schizophrenia, schizoaffective disorder, or other psychotic disorders. Hoch (1951) demonstrated exacerbation of symptoms in patients with latent schizophrenia following intravenous mescaline.
- family history of psychosisabsolute
First-degree family history of schizophrenia or psychotic disorders. Risk of precipitating a first psychotic episode in genetically predisposed individuals.
- bipolar disorderrelative
Bipolar I and II disorders. Risk of triggering manic or mixed episodes.
Pregnancy & Breastfeeding

Other
- concurrent MAO inhibitor useabsolute
Concurrent use of MAOIs. Creates both pharmacokinetic (increased exposure) and pharmacodynamic (serotonin syndrome) risk.

Concurrent lithium carbonate therapy. Case reports for LSD/psilocybin; no mescaline-specific reports but shared 5-HT2A mechanism.
