Contraindications

ConditionSeverityNote
Respiratory
Respiratory insufficiencyrelative

Any pre-existing respiratory compromise increases the risk of clinically significant respiratory depression with etizolam. Class-level contraindication for all benzodiazepine-site agonists.

Sleep apnearelative

Benzodiazepine-class contraindication. GABA-A positive allosteric modulation reduces upper airway muscle tone and central respiratory drive, worsening both obstructive and central sleep apnea. No etizolam-specific sleep apnea data; inferred from benzodiazepine class evidence.

Neurological
Myasthenia gravisabsolute

Benzodiazepine-class contraindication. GABA-A positive allosteric modulation reduces motor neuron excitability, worsening the already-compromised neuromuscular transmission in myasthenia gravis. No etizolam-specific data; inferred from diazepam and other benzodiazepine class evidence.

Psychiatric
History of substance use disorderrelative

Etizolam's high binding affinity and rapid onset predict strong interoceptive reinforcement cues. Tamburin et al. (2020) documented prescription-to-dependence trajectories in 11 patients originally prescribed etizolam for medical indications, all of whom escalated to supratherapeutic doses.

Metabolic
CYP2C19 poor metabolizer genotype with concurrent CYP3A4 inhibitorrelative

CYP2C19 poor metabolizers (3–5% European, 15–20% East Asian populations) already experience 2.65-fold higher AUC. Adding a CYP3A4 inhibitor (itraconazole, ketoconazole, clarithromycin, HIV protease inhibitors) increases AUC to 6.18-fold above normal. If co-administration is clinically necessary, substantial dose reduction is required. This is an etizolam-specific pharmacogenomic interaction quantified by Yamamoto et al. (2017).

Pregnancy & Breastfeeding
Pregnancyrelative

Benzodiazepine class contraindication. Associated with neonatal withdrawal, floppy infant syndrome, and possible teratogenic effects. No etizolam-specific teratogenicity or reproductive toxicity data have been published.

Age
Elderly patientsrelative

Elderly patients show increased sensitivity to CNS depressant effects, higher fall risk, and potentially slower CYP-mediated metabolism. NCT00707915 specifically investigated cognitive and motor effects of benzodiazepine-derivative discontinuation in elderly patients.

Other
Concurrent opioid useabsolute

The combination of etizolam with opioids — particularly fentanyl and its analogues — produces synergistic CNS and respiratory depression and is the primary driver of etizolam-associated mortality. In Ontario (2017–2021), etizolam was the most frequently detected benzodiazepine in 11,033 opioid-positive death investigations. This combination is never safe at any dose.

Concurrent alcohol useabsolute

Alcohol and etizolam both enhance GABAergic inhibitory neurotransmission, producing additive CNS and respiratory depression. Class-level contraindication; no etizolam-specific controlled data, but the mechanism is pharmacologically unambiguous.

Acute angle-closure glaucomaabsolute

Benzodiazepine-class contraindication. Mydriatic effects associated with benzodiazepine-site agonists may worsen or precipitate acute angle-closure glaucoma. No etizolam-specific ophthalmological data; inferred from benzodiazepine class evidence.
