Uncontrolled hypertension or history of hypertensive crisis. αMT's monoamine releasing activity produces dose-dependent cardiovascular stimulation.

History of cardiac arrhythmia or structural heart disease. The sustained cardiovascular stimulation over 13–15 hours represents prolonged cardiac stress.

History of epilepsy or seizure disorders. Seizures have been documented in clinical toxicity reports of αMT.

Personal or family history of schizophrenia, schizoaffective disorder, or other psychotic spectrum disorders. 5-HT₂A agonists are established precipitants of psychotic episodes in vulnerable individuals.

Personal or family history of bipolar I or II disorder. Both the psychedelic and stimulant components of αMT's mechanism can destabilise mood cycling.

Significant hepatic impairment may alter αMT metabolism. Clinical significance is unknown due to absence of CYP isoform data.

Pregnancy and breastfeeding are absolute contraindications in the absence of any reproductive safety data.

Current use of any serotonergic medication including SSRIs, SNRIs, MAOIs, lithium, triptans, tramadol, or St John's Wort. A washout period of at least 5 elimination half-lives of the serotonergic medication is required.
