Cardiovascular

Serotonergic psychedelics can trigger acute psychotic episodes or worsen existing psychotic illness. This is a class-level absolute contraindication documented across LSD and psilocybin literature (Goldberg et al. 2020). No AL-LAD-specific data, but mechanism is shared.
- Anxiety and mood disordersrelative

Individuals with anxiety or mood disorders face increased risk of acute distress, panic, and challenging psychological experiences. Pre-existing anxiety disorders are also identified as a risk factor for HPPD (Halpern et al. 2018). This is a relative rather than absolute contraindication — therapeutic psychedelic use in mood disorders is an active research area for other serotonergic psychedelics.
Pregnancy & Breastfeeding

No reproductive or developmental toxicity data have been published for AL-LAD. In the absence of safety data, psychoactive substance use during pregnancy is contraindicated as a precautionary measure.
Other
- Lithiumabsolute
Concurrent lithium use with serotonergic psychedelics is associated with increased seizure risk at the class level. No AL-LAD-specific case data exist, but this interaction is consistently reported for LSD. The combination is classified as an absolute contraindication due to seizure severity.
- Tramadolrelative
Tramadol inhibits serotonin reuptake and lowers seizure threshold. Combined with AL-LAD's serotonergic agonism, this creates risk of serotonin syndrome and seizures. Park et al. (2014) documented serotonin syndrome risk when tramadol is combined with serotonergic agents. No AL-LAD+tramadol case reports exist.
- MAOIsrelative
MAOIs block serotonin metabolism, potentially causing dangerous serotonin accumulation when combined with AL-LAD's direct serotonergic agonism. This is a well-established class-level interaction for serotonergic drugs. No AL-LAD-specific case data.
- SSRIs/SNRIsrelative
SSRIs and SNRIs can both reduce AL-LAD's psychedelic effects and increase serotonin syndrome risk. The net effect is unpredictable without controlled study. Users on chronic SSRI/SNRI therapy should be aware of both diminished efficacy and increased safety risk.
