Contraindications

ConditionSeverityNote
Cardiovascular
cardiovascular diseaserelative

Relative contraindication in individuals with hypertension, coronary artery disease, arrhythmias, or heart failure. Cardiovascular stress is a class effect of arylcyclohexylamines.

Respiratory
respiratory insufficiencyabsolute

Any condition impairing respiratory function (COPD, severe asthma, sleep apnea) increases overdose risk. Respiratory depression confirmed as cause of death in polydrug case involving 3-MeO-PCE.

Neurological
epilepsyabsolute

Contraindicated in individuals with epilepsy or seizure disorders. Also contraindicated in combination with other medications that lower seizure threshold.

Psychiatric
psychotic disordersabsolute

Strongly contraindicated in individuals with personal or family history of psychotic disorders including schizophrenia and schizoaffective disorder. The dual NMDA antagonism and serotonergic potentiation creates an elevated psychotomimetic profile relative to other dissociatives.

bipolar disorderabsolute

Strongly contraindicated. 3-MeO-PCE carries higher mania risk than most dissociatives due to its potent serotonin reuptake inhibition layered onto NMDA antagonism.

Hepatic
hepatic impairmentrelative

Relative contraindication. Hepatic metabolism confirmed (Xu et al. 2024) but specific CYP enzymes not identified. Dose adjustment cannot be recommended without pharmacokinetic data.

Pregnancy & Breastfeeding
pregnancy and breastfeedingabsolute

No reproductive toxicity data exist for 3-MeO-PCE. Contraindicated on a precautionary basis given NMDA receptor involvement in neurodevelopment and absence of safety data.

Other
concurrent serotonergic medicationabsolute

Absolutely contraindicated with MAOIs. Strongly contraindicated with SSRIs and SNRIs. Fatal case involving SERT-inhibiting arylcyclohexylamine (N-ethyldeschloroketamine) combined with venlafaxine reported.
