Contraindications

Respiratory

  • Respiratory insufficiencyabsolute

    2M2B causes dose-dependent respiratory depression via GABA-A potentiation of brainstem respiratory centers. Two published cases document respiratory failure requiring emergency intubation (Anand 2014; Diaz 2024). Pre-existing respiratory insufficiency, COPD, sleep apnea, or any condition reducing baseline ventilatory reserve substantially increases the risk of fatal respiratory arrest.

Neurological

  • Seizure disorderrelative

    Chronic GABA-A PAM exposure produces compensatory NMDA receptor upregulation and increased glutamatergic signaling. Abrupt cessation or even the natural offset of a single high dose can unmask this latent hyperexcitability, lowering seizure threshold. Individuals with pre-existing seizure disorders face elevated risk of breakthrough seizures both during withdrawal and during the offset phase of acute intoxication.

Psychiatric

  • History of alcohol or sedative dependencerelative

    Individuals with a history of alcohol or sedative dependence face compounded risk: cross-tolerance requires higher doses for subjective effect while respiratory depression sensitivity is not proportionally reduced, narrowing the safety margin. 2M2B's undetectability on immunoassay screens (confirmed in all published case reports) enables use that evades monitoring in treatment and legal contexts. The e-psychonauts case study (2022) documents 2M2B addiction in an individual with prior substance dependence history.

Hepatic

  • Hepatic impairmentrelative

    Unlike ethanol, which is primarily cleared by cytosolic ADH, 2M2B depends entirely on CYP-mediated oxidation for elimination. Hepatic impairment from cirrhosis, hepatitis, or other liver disease would reduce clearance capacity, potentially extending the already long 8–14 hour intoxication window and increasing cumulative toxicity. TAME rat studies showing up to 260-fold CYP2B1 induction suggest chronic exposure itself may stress hepatic function.

Pregnancy & Breastfeeding

  • Pregnancyrelative

    Ethanol is an established teratogen producing fetal alcohol spectrum disorders through GABA-A potentiation and NMDA antagonism during neurodevelopment. 2M2B shares both mechanisms and, as a small lipophilic molecule, is expected to cross the placental barrier readily. No animal or human reproductive toxicity studies have been conducted for 2M2B. The presumption of teratogenicity is based entirely on structural and mechanistic analogy with ethanol.

Other

  • Concurrent CNS depressant useabsolute

    Co-administration of 2M2B with opioids, benzodiazepines, barbiturates, ethanol, or GHB produces synergistic CNS and respiratory depression that may be lethal. Published case reports note that 2M2B users are frequently polysubstance-dependent individuals who combine the compound with other depressants (Syed & Leo 2022; Anand 2014), making this interaction clinically documented rather than purely theoretical.
