Dose-dependent cardiovascular pressor effects documented in controlled studies. Uncontrolled hypertension, severe arrhythmias, or structural heart disease represent absolute contraindications due to added hemodynamic stress.

Pre-existing seizure disorders or epilepsy. Seizures documented in severe 2C-class toxicity.

Personal or family history of schizophrenia, schizoaffective disorder, or other psychotic spectrum conditions. 5-HT2A agonist psychedelics carry class-level risk for psychosis precipitation.

Personal or family history of bipolar I or II disorder. Risk of manic episode precipitation. Lithium co-administration creates additional interaction risk.

Pregnancy and breastfeeding are absolute contraindications. No reproductive or developmental toxicology data exist for 2C-B.

Concurrent use of any monoamine oxidase inhibitor is absolutely contraindicated due to expected dramatic increase in 2C-B systemic exposure.

Concurrent lithium therapy. Mechanism incompletely characterized but includes potential seizure threshold reduction and altered serotonergic signaling.
