MDMA Supplementation Protocol

Evidence-based supplementation protocol for reducing oxidative stress and neurotoxicity associated with MDMA use. Based on published research on MDMA metabolism pathways.

Critical: 5-HTP must NOT be taken on the same day as MDMA. Taking a serotonin precursor with a serotonin releaser risks serotonin syndrome, a potentially fatal condition.

pre-loading (days before)

N-Acetyl Cysteine (NAC)Days before, stop day-of
600 mg, 1–2x daily in the days before
purpose — Precursor to glutathione, the body’s primary antioxidant. Reduces oxidative stress from MDMA metabolism.
evidence — Animal studies show significant neuroprotection. NAC may reduce subjective effects if taken day-of — stop the morning of use.
Alpha Lipoic Acid (ALA)Before and during
100 mg every 1–2 hours
purpose — Antioxidant that crosses the blood-brain barrier. Helps neutralize free radicals generated by MDMA metabolism.
evidence — R-ALA form is more bioactive. Has been shown to prevent MDMA-induced neurotoxicity in animal models (Aguirre et al., 1999).
Vitamin CBefore and during
1,000 mg before, 500 mg during
purpose — Water-soluble antioxidant. Scavenges free radicals.
evidence — Supportive antioxidant role. Well-tolerated at these doses.
Magnesium GlycinateBefore and during
200–400 mg
purpose — NMDA receptor antagonist at this dose level. Reduces jaw clenching (bruxism), muscle tension, and may be neuroprotective.
evidence — Widely reported to reduce bruxism. Glycinate form has best absorption and least GI distress.

during

Electrolyte drinksDuring
Sip steadily, don’t chug
purpose — MDMA increases water retention (SIADH). Drinking plain water can cause hyponatremia (dangerously low sodium). Electrolytes maintain sodium balance.
evidence — Hyponatremia is a documented cause of MDMA-related death. Electrolyte replacement is standard medical advice.
Alpha Lipoic Acid (continued)Every 1–2 hours during
100 mg
purpose — Continue antioxidant protection through the period of peak oxidative stress.
evidence — See pre-loading notes.

post-loading (days after)

5-HTP + EGCGNext day and following days (NOT same day as MDMA)
100 mg 5-HTP + 400 mg EGCG, 1–2x daily for 3–7 days
purpose — 5-HTP is a serotonin precursor that helps replenish depleted serotonin stores. EGCG (from green tea extract) ensures 5-HTP converts to serotonin in the brain rather than the gut.
evidence — 5-HTP is widely used for post-MDMA serotonin recovery. MUST NOT be taken same day as MDMA or with other serotonergics due to serotonin syndrome risk. EGCG as a peripheral decarboxylase inhibitor is supported by Hinz et al.
N-Acetyl Cysteine (resumed)Resume day after
600 mg daily for 1 week
purpose — Continue glutathione support during recovery period.
evidence — See pre-loading notes.
Vitamin CDays after
500–1,000 mg daily
purpose — Continue antioxidant support during recovery.
evidence — Supportive role.

why supplementation helps

MDMA neurotoxicity is primarily caused by oxidative stress from MDMA metabolites, not by MDMA itself. The metabolic pathway produces reactive species (MDA, alpha-methyldopamine, and subsequent quinones) that damage serotonergic neurons.

Antioxidants (NAC, ALA, vitamin C) neutralize these reactive species. Magnesium blocks NMDA receptors involved in excitotoxicity. 5-HTP (post-session only) replenishes depleted serotonin stores.

However, the most important harm reduction factors for MDMA are: dose (lower is safer), frequency (3+ months between uses), and temperature (stay cool). Supplementation is the third line of defense, not the first.

critical warnings

–Do NOT take 5-HTP on the same day as MDMA. Combining a serotonin precursor with a serotonin releaser risks serotonin syndrome.
–Do NOT take 5-HTP if you are on SSRIs, SNRIs, MAOIs, or other serotonergic medications without medical supervision.
–Supplements reduce harm but do not eliminate it. MDMA neurotoxicity is dose-dependent — lower doses and longer intervals between uses are the most important factors.
–The recommended minimum interval between MDMA uses is 3 months (the “3-month rule”). More frequent use increases neurotoxicity risk regardless of supplementation.
–Body temperature is a major factor in MDMA neurotoxicity. Stay cool, take breaks from dancing, and monitor for hyperthermia.
–These supplement recommendations are based on published research but are not FDA-approved for this purpose. Consult a healthcare provider if you have underlying health conditions.
